OSHA issues 16 citations following fatal sewer confined space incident
The U.S. Department of Labor has cited an Alabama-based contractor following a fatal confined space incident involving sewer line work in Mobile, Ala., that resulted in two worker deaths.
According to the Occupational Safety and Health Administration (OSHA), three employees of Construction Labor Services Inc. were installing and repairing sewer lines inside a manhole on Aug. 11, 2025, when they were overcome by sewage gases. One worker regained consciousness and was able to exit the confined space, while the other two workers died.
Federal investigators determined the employer failed to implement required confined space entry programs, including procedures to evaluate atmospheric conditions, provide worker training and establish emergency response protocols. OSHA cited the company for 16 serious violations and proposed penalties totaling $257,707.
OSHA officials said the case underscores the ongoing hazards associated with sewer and manhole work, particularly exposure to toxic or oxygen-deficient atmospheres in confined underground spaces. The agency noted that employers must ensure proper atmospheric testing, ventilation, rescue planning and worker training when performing sewer line and utility construction activities.
